What actually gets smaller
Photographs and scanned pages. Those are where the megabytes live, and re-encoding them is where the saving comes from. A ten-megabyte scan commonly drops by 80–90%.
A text-only PDF will barely change, and that is correct — there is nothing to compress. If your file does not shrink much, it was already efficient.
What is left alone
Text stays selectable and stays sharp. It is never rasterised. Vector logos and line art are untouched. Images with transparency are skipped rather than flattened, and any image using a colour format we do not fully understand is copied through unchanged rather than guessed at.
This is lossy, so keep the original
Re-encoding a photo throws away detail permanently. Compress a copy for sending, keep the original for archiving and printing. That is true of every compressor, including the paid ones.
